Project Infinity Reach OUT

A Daily Call Service that contacts people in our ageing community to check in, exchange a friendly greeting and conversation and see if they are happy and well.
With growing isolation and loneliness experienced as we age, coupled with physical and mental illness, this Reach OUT service is a preventative health service that focuses on creating connection, care, understanding and early alerts for those who may be home alone or not be in daily contact with their family, friends and extended community.
This will provide peace of mind to our ageing community and their immediate, extended family and friends.

- Daily check in monitoring for happiness and wellness of our ageing community
- Early detection of illness, health concerns or accidents
- Early response to injury or illness that may be treatable through earlier assessment, care/hospitalisation
- Early alert process including calling approved neighbour, family and emergency services

- Increased connectivity with community for those who are otherwise less able
- Greater care and support for Positive Ageing and staying at home for life
