Free program open to all men and women (18+) open to all surrounding communities. 16 week program to be conducted in the third largest corridor and third fastest growing local area in Victoria; low socioeconomic population, one third are immigrants or refugees, low rate of community acceptance of diverse cultures. We have identified chronic disease, mental health and preventive health care as priorities
for the Outer North community. Program involves: 26 weeks x 2 hour sessions, covering Physical activities, nutrition, vascular health, weight management, Bone and Mental Health. - conclusion of program, opportunity for participants to become community leaders.

Men and women play a significant role in the community and are central to cultural change in their environments. The program design is to bring all together from various age groups, cultures and socioeconomic backgrounds to engage in an environment that promotes health, wellbeing and community connectedness extending to the family circle including children. Key goals of the program are:
Health literacy, chronic disease management, promoting and engaging in physical exercise, enhancing community social connectedness by reducing isolation and encouraging community acceptance, cohesion and resilience. Data will be collected to monitor benefits to participants; evaluated and reported with recommendations.
